READ BEFORE SIGNING
RELEASE OF ALL CLAIMS AND WAIVER OF LIABILITY
Participants under 18 must have parental consent
and must have the waiver signed in advance.
I AGREE TO THIS RELEASE OF CLAIMS AND WAIVER OF LIABILITY.
I understand that rock climbing, mountaineering, ice climbing and outdoor excursions which
include safety and rescue procedures, backpacking and outdoor and indoor team building trips
involve dangers, not all of which can be listed here. Among the more obvious and frequent are:
1. Steep terrain where a fall, whether roped or unroped, may cause injury and/or death;
2. Unstable or loose rock/ice, and/or rockfall/icefall which may cause injury and/or death;
3. Slippery terrain including ice, snow, mud, rocks, rubble and roots which may cause a fall
resulting in injury and/or death;
4. Failure of climbing and/or rescue equipment despite reasonable care and use which could result
in injury and/or death;
5. Careless or reckless behavior on the part of other members of the group despite reasonable
supervision; and careless or reckless behavior on the part of third parties.
I accept all of the risks and the possibility of death, personal injury, property damage and
loss resulting from my involvement with the trip and/or course I am taking with 2009901 Ontario
Inc., operating as Muskoka Outfitters.
I forever release 2009901 Ontario Inc., its officers, employees, guides, agents and
representatives from any and all liability for any cause whatsoever including negligence on the part
of 2009901 Ontario Inc., its officers, employees, guides, agents or representatives.
I certify that I have consulted with a doctor with regards to my health any medical
conditions I may have, and am physically capable and fit to participate in this activity.
I agree to wear a helmet and it is my responsibility to use it properly.
I confirm that I am eighteen years of age or older. (Younger participants must have a
parent or guardian read and sign this document.)
I confirm that I have read over this agreement before signing, that I understand it, and
that it will be binding not only on me but also on my heirs, my next of kin, my executors,
administrators and assigns.
I agree that the laws of the Province of Ontario govern this contract.
I HAVE READ THIS DOCUMENT THOROUGHLY AND UNDERSTAND IT.
I UNDERSTAND THAT BY SIGNING THIS DOCUMENT I GIVE UP SUBSTANTIAL LEGAL RIGHTS I
WOULD OTHERWISE HAVE.
I SIGN THIS DOCUMENT VOLUNTARILY AND WITHOUT INDUCEMENT
